transparent bundled payment options for patients and employers seeking to pay directly for
their orthopedic care outside of an in-network insurance plan. Our transparent bundles are
perfect for:

• Patients without insurance (self-pay)
• Self-insured employers
• Patients with insurance but are out-of-network
• Patients with high-deductible plans
• International patients
• Members of faith-based sharing ministries

Please note that if you have insurance, choosing one of these self-pay packages
waives your right to send the charges associated with the appointment to insurance.

submission to insurance for reimbursement or coverage. This package is designed
specifically for individuals who choose to pay for their medical services out-of-pocket
and is not intended for submission to any third-party payers, including insurance
companies.
